We are pleased to welcome you to our practice. We are aware that you lead busy lives and have made the new patient forms available for you to fill out and bring to your next appointment to save time. The following information is necessary to enable us to provide you with the best dental care and all the information disclosed is confidential. New patient and privacy policy forms are required for every new patient. As new patient forms contain essential medical information for our dental practitioners to provide your treatment, they do need to be updated every 12 months in case of any changes. We have both an English and Vietnamese version for your convenience. Any children eligible for the Child Dental Benefits Schedule will require an additional CDBS consent form.

If you are unable to print out the above forms, we have them available at our clinic for you to complete. We ask that you arrive 10 minutes prior to your appointment to complete these forms.
